These forms can be printed and faxed to us at 630.904.7987. They also can be mailed or dropped off in person during Narnia's office hours.

Registration Form

Use the registration form to sign up for new or continuing classes. Please note that sending in the registration form does not guarantee you a spot in class. Our classes fill up very quickly. Your credit card will not be charged until you have a confirmed spot in class. If you would like to pay with cash or check, please make arrangements with us by calling 630.904.0895.

Health Certificate

The Health Certificate is required before starting class. It should be completed by your veterinarian and certify that your dog is ready to start class.

Orientation Questionnaire

The new client questionnaire is completed during orientation or if you prefer, you can download it and get a head start. Be sure to bring it with you to orientation.

Refund Policy

If you choose to cancel your registration we must be notified in writing at least 7 days before the start of your class. No refunds will be issued for cancellations received less than 7 days before the start of class.
